Burns: assessment and Parkland fluids

First minutes: stop the burning, protect the airway

Remove the patient from the source; remove burnt clothing and all jewellery (fingers swell fast). Cool the burn under running water for 20 minutes — effective within 3 hours of injury — while keeping the rest of the patient warm and dry. Never apply ice, toothpaste, egg or soil: they deepen injury and cause infection.

Screen the airway immediately in every flame or enclosed-space burn: facial burns, singed nasal hair, soot in the mouth, hoarseness or stridor mean inhalation injury — swelling worsens for hours, so what is a voice change now can be complete obstruction by the time the ambulance is on the highway. Escalate and refer these patients early, sitting up, with the most experienced person watching the airway. After enclosed-space fires suspect carbon monoxide; the SpO2 monitor may read falsely normal — give high-flow oxygen regardless.
